Your HealthSmart Member ID is located with your name on the front of your ID card. Please make sure to include all characters listed when entering your Member ID at Tava.

Tava can see HealthSmart members whose ID cards list the following payers on the back of the card for claims management:

• First Health

• Aetna

• Cigna

• ClaimDOC

• UnitedHealthcare

Check to see if your employer provides coverage for Tava. Additionally, you can always pay cash for Tava services. Some insurance plans allow you to see a provider that is “out of network” and receive full or partial reimbursement. Ask us for a quarterly Superbill to submit out-of-network insurance reimbursement claims by emailing client-support@tavahealth.com.
