Your Baylor Scott and White Member ID is located beside your name on the front of your ID card. Please make sure to include all characters listed when entering your Member ID at Tava.
Please note: When entering your ID number onto your profile, please enter the ID number for the person the profile is for. With Baylor Scott and White policies, each member on the plan gets a different ID. In order for our e-checks to work, we have to have the ID of the person the profile is for and have them listed as the subscriber (even if the client is a dependent on the plan.)
PLEASE NOTE: We CANNOT SEE clients with any CHIP, Medicaid, Medicare, or Medicare Advantage plans at this time, including as a secondary payer for services.
Check to see if your employer provides coverage for Tava. Additionally, you can always pay cash for Tava services. Some insurance plans allow you to see a provider that is “out of network” and receive full or partial reimbursement. Ask us for a quarterly Superbill to submit out-of-network insurance reimbursement claims by emailing support@tavahealth.com.
