If you are a Clearlink employee utilizing Tava's EAP, you have the option to use your insurance policy to cover your sessions after you have used all of your allotted sessions through Clearlink EAP.
If you are a client who resides in the state of Utah, the policy you need to upload to your Tava profile is through Select Health.
If you are a client who resides outside the state of Utah, the policy you need to upload to your Tava profile is through UnitedHealthcare Shared Services.
Please enter in your member ID as printed on your insurance card, including all letter and number characters. If you are the subscriber, please enter your name and date of birth as your insurance company has it in their records. If someone else is the subscriber on the policy, please enter their name, date of birth, and your relationship to this person.
