Your mental health assessment scores provide insight into anxiety, depression, and overall mental distress. These scores help track changes over time and guide discussions with your therapist.
Each question in the assessment is scored from 0 to 3, with total scores categorized as follows:
Anxiety (GAD-7)
Used to assess generalized anxiety symptoms over the past two weeks.
• 0–4: Minimal Anxiety
• 5–9: Mild Anxiety
• 10–14: Moderate Anxiety
• 15+: Severe Anxiety
Depression (PHQ-9)
Measures depressive symptoms, including mood, energy levels, and concentration.
• 0–4: Minimal Depression
• 5–9: Mild Depression
• 10–14: Moderate Depression
• 15–19: Moderately Severe Depression
• 20–27: Severe Depression
What Do These Scores Mean?
These scores are not a diagnosis but rather a tool to help you and your therapist understand patterns in your mental health. Your therapist can help interpret your results and guide next steps, whether that means adjusting your therapy plan, exploring new coping strategies, or seeking additional support.
