Your Personify Health Member ID is located below your name on the front of your ID card. Please make sure to include all characters listed when entering your Member ID at Tava.
Please note that we can only see client's whose Personify Health insurance uses the Aetna or Cigna network. You will see the Aetna or Cigna logo under "medical plan" on the front of your ID care as presented below.
We are unable to see client's through insurance with Personify Health that uses any other network at this time. Clients are welcome to see us via cash pay.
PLEASE NOTE: We CANNOT SEE clients with any CHIP, Medicaid, Medicare, or Medicare Advantage plans at this time, including as a secondary payer for services.
Check to see if your employer provides coverage for Tava. Additionally, you can always pay cash for Tava services. Some insurance plans allow you to see a provider that is “out of network” and receive full or partial reimbursement. Ask us for a quarterly Superbill to submit out-of-network insurance reimbursement claims by emailing support@tavahealth.com.
