Your MotivHealth Member ID is located on the left side of your ID card under the subscriber's name. Please be sure to include all characters when uploading the ID to your Tava profile.
PLEASE NOTE: We CANNOT SEE clients with any CHIP, Medicaid, Medicare, or Medicare Advantage plans at this time, including as a secondary payer for services.
Check to see if your employer provides coverage for Tava. Additionally, you can always pay cash for Tava services. Some insurance plans allow you to see a provider that is “out of network” and receive full or partial reimbursement. Ask us for a quarterly Superbill to submit out-of-network insurance reimbursement claims by emailing support@tavahealth.com.
