Your BlueCross BlueShield (BCBS) of Vermont Member ID is located below your name on the front of your BCBS ID card.  Please make sure to include your three-digit alpha prefix, as well as all of the numbers after these when entering your Member ID at Tava.

At this time, we are unable to see clients with the following BCBS of Vermont Plans:  

• Vermont Blue Advantage Plus Freedom and Freedom Plus PPO (PPO) 
• Vermont Blue Advantage Tribute PPO (PPO) 
• Vermont Blue Advantage Unity HMO (HMO)

PLEASE NOTE: We CANNOT SEE clients with any CHIP, Medicaid, Medicare, or Medicare Advantage plans at this time, including as a secondary payer for services.
