You can find your Anthem Blue Cross Blue Shield (BCBS) Member ID on the left side of the front of your insurance card. The first three characters will include some letters and numbers. Please include all of letters and numbers when entering you Anthem BCBS Member ID at Tava.
PLEASE NOTE: We CANNOT SEE clients with any CHIP, Medicaid, Medicare, or Medicare Advantage plans at this time, including as a secondary payer for services.
If you don't have your insurance card, all registered Anthem members can also access your Anthem ID digitally when you download the SydneySM Health app or log in to your Anthem account.
Check to see if your employer provides coverage for Tava. Additionally, you can always pay cash for Tava services. Some insurance plans allow you to see a provider that is “out of network” and receive full or partial reimbursement. Ask us for a quarterly Superbill to submit out-of-network insurance reimbursement claims by emailing support@tavahealth.com.
