Your BlueCross BlueShield (BCBS) of Illinois Member ID is located below your name on the front of your BCBS ID card. Please make sure to include your three-digit alpha prefix, as well as all of the numbers after these when entering your Member ID at Tava.
At this time, we are ABLE to see clients with the following BCBS of Illinois Commercial Plans:

- Blue Choice PPO
At this time, we are UNABLE to see clients with the follow BCBS of IL Commercial Plans:
- HMO Medical Groups (MG)
- HMO Independent Practice Associations (IPA)
- HMO PNC for Medical Groups (MG) and Independent Practice Associations (IPA)
- Blue Advantage HMO
- Blue Precision HMO
- BlueCare Direct
- Blue FocusCare
At this time, we are UNABLE to see clients with the following BCBS of IL Government Plans:
- Blue Cross Community Health Plans (BCCHP)
- Blue Cross Community MMAI (Medicare-Medicaid)
- Medicare Advantage – Blue Cross Medicare Advantage (HMO)
- BlueCross Medicare Advantage HMO
- Blue Cross Medicare Advantage (PPO)
Check to see if your employer provides coverage for Tava. Additionally, you can always pay cash for Tava services. Some insurance plans allow you to see a provider that is “out of network” and receive full or partial reimbursement. Ask us for a quarterly Superbill to submit out-of-network insurance reimbursement claims by emailing support@tavahealth.com.
PLEASE NOTE: We CANNOT SEE clients with any CHIP, Medicaid, Medicare, or Medicare Advantage plans at this time, including as a secondary payer for services.
